Please let us know if you or your loved one is going out to an appointment so we can plan ahead of time to send paperwork and documentation that is needed for that appointment.
Remember to stop at the nurses’ station before you leave for the appointment to pick up your paperwork. Also, if you would like The Neighbors staff to assist in setting up transportation to/from an appointment, we need to know well ahead of time. We do not have a transportation company ourselves and need to look for assistance elsewhere. There is generally a charge, so please ask ahead of time.
When we do send a consult with you to the appointment, we also need to get it back so we know if there were order changes or not. Please give this to the nurse upon your return to the facility. This will help us in continuity of care and giving better care to your loved one/you.
